Wisconsin Application for Absentee Ballot

If you are not already registered, you will need to complete a Voter Registration Application before an absentee ballot can be sent to you.

Ballots will be available between the third Monday before Election Day up until 5 pm on the Friday before the election.

You will be required to show Photo ID the first time you vote, unless you move or change your name, when requesting a ballot in-person or by mail.

Deadline for Returning a Ballot

The completed ballot must be received by the Town Clerk no later than the day of the election so that it can be delivered to the polling location by 8 pm.

NOTE: There are exceptions for military and overseas voters.

How to Apply for an Absentee Ballot

Request by Mail: Submit a written request by completing the Wisconsin Application for Absentee Ballot no later than 5 pm on the 5th day before the election. SIGN and DATE the application.

Request by E-Mail or Fax: Before the ballot can be counted on Election Day, the Clerk must receive a request from the voter with an original signature. (Print your e-mailed request, sign and return it with your ballot, but outside the certified ballot envelope) no later than 5 pm on the Thursday before the election in order for an absentee ballot to be mailed to you.

[Exception: for Military and indefinitely confined who must submit their request no later than 5 pm on the Friday before the election.]

Request in Person: Town Clerk’s Office, 2000 Municipal Drive (County Highway CB & E. Shady Lane), up until 5 pm on the Friday before the election.

Note: When applying for an absentee ballot in the Clerk’s Office, you must vote immediately, seal your ballot in the proper envelope, and return it to a member of the Clerk’s staff. No ballots may be taken from the Clerk’s Office.

Absentee voters may include:

  • Permanent Absentee: You may be placed on a permanent absentee list if indefinitely confined because of age, illness, infirmity or disability. Complete the Wisconsin Application for Absentee Ballot and under number 6, check the 3rd choice. You will be automatically provided a ballot for every election until such time as you notify the Clerk, or fail to return an absentee ballot at that election. If you choose not to vote at an election, you must notify the Clerk’s office or mail back your unvoted ballot; otherwise you will be removed from the permanent list to receive a ballot and must re-apply.
  • Hospitalized Elector: A hospitalized elector who is registered can choose a person as an agent to apply for and obtain the absentee ballot. The agent shall present any proof of identification required when submitting the Wisconsin Application for Absentee Ballot. The request may be filed at the Clerk’s Office not earlier than 7 days before an election and not later than 5 pm on Election Day. Applicant must supply the required information and must sign the application. After the completed absentee ballot is sealed by the hospitalized voter, it is returned to the Clerk either by mail or by personal delivery of the agent; but if the ballot is returned on the day of the election, the agent must personally deliver the ballot to the Clerk or to the polling place serving the hospitalized elector’s residence by 8 pm on election day.
  • Confidential Listing: You may be placed on the state’s confidential voting list. Your personal information will be suppressed on any poll/registration list and withheld from public inspection. Certain criteria must be presented in person to the Town Clerk.
